Getting on board with DESMOND


DESMOND is the name of a family of programmes designed to support self-management in diabetes care. There are a number of stand-alone modules that can be adopted by your health organisation to help build a local profile to support self management. 

Together, DESMOND modules can help you build a comprehensive pathway of education integrated with clinical management across the whole spectrum of Type 2 diabetes. There are programmes to support people at risk, those who are newly diagnosed, and those with established diabetes. There are new modules being developed through our partnership with internationally renowned diabetes research teams, specifically our Ongoing with diabetes module for long term self-management in diabetes.

 

Our full list of programmes is constantly growing:

 

  • Newly Diagnosed DESMOND for people at the start of their journey with Type 2 diabetes
  • Foundation – for people with established diabetes
  • BME (Black and minority ethnic) DESMOND – Newly Diagnosed and Foundation programmes culturally specific for people of South Asian origin.
  • Walking Away – a module for those ‘at risk’ of diabetes
  • Ongoing DESMOND - a model of care integrating self management education with clinical management
  • DESMOND for those with a learning disability is currently under development
  • A Safer Ramadan – a DESMOND module under development to support people making decisions about fasting during this month

 

Making DESMOND an integrated part of your overall strategy for diabetes is the first step in achieving an effective self management plan for your patients.

 

The DESMOND National Office can provide help and support should you want to explore how hosting DESMOND can fit into your strategy for self-management. DESMOND is an NHS initiative, and is therefore committed to providing a cost effective strategy to meet individual organisational needs.
