The Yorkshire and Humber Public Health Observatory have a commitment to support the diabetes community by providing timely, quality assured national diabetes health intelligence.
They work closely with a range of key stakeholders including NHS Diabetes, Strategic Health Authorities, Primary Care Trusts, Diabetes UK, The Information Centre and many others.
They are also a strong partner in the National Diabetes Information Service (NDIS) with a vision of providing one portal for all diabetes related information. They are part funded by NHS Diabetes to provide an annual work programme which includes analysing and interpreting the kind of facts, figures and other intelligence that will help to improve the health of the diabetes community
The Diabetes Community Health Profiles bring together a wide range of data on diabetes in adults into a single source for the purposes of benchmarking. It provides data relating to adults and children with diabetes. It is designed to provide an overview of the key areas of diabetes care and highlight issues for further investigation.

The tool allows you to download a diabetes profile for each PCT in England.
In order to locate the PCT profile you require please select a Strategic Health Authority from the drop down list. The tool will then generate a list of PCTs within that Strategic Health Authority.
