Local referral pathway to DESMOND locally: Via GP Practice.
Bedfordshire PCT currently has 2 teams of trained educators/facilitators who cover the whole County i.e. 1 Diabetes Specialist Nurse; 3 Diabetes Dietitians and 3 Practice Nurses. Each programme is delivered either as a 6 hour session or as two 3 hour sessions by 2 of the educators/facilitators. Referrals are accepted from all GP Practices for patients that have been diagnosed with Type 2 diabetes within the past 6 months. A centralised administration system has been established within the Health Improvement Team to coordinate the sessions and attendance.
