How to order your DESMOND Resources

The New DESMOND Order Form
This now includes all of the new resources that have been introduced with the new curriculum.
Dedicated Email Address
We now have an email address dedicated to resource orders desmond.orders@uhl-tr.nhs.uk
Please ensure that all orders are now sent to this address.
Delivery Costs
Delivery costs have now been included in the order form (still £11.75 per order).
Ordering
All quantities are on drop down menus as per the old form.
Please note that the patient booklet is currently under revision & will be available to purchase from Monday 7th June. Until then we have a limited stock of the current version. The changes to the booklet are purely cosmetic and therefore all current versions will still be valid.
Free of Charge Resources
If you have not yet claimed your free of charge resources, please can you now order these through our new email address desmond.orders@uhl-tr.nhs.uk.
The resources you are entitled to claim are:
- Physical Activity Cards - These are a new resources
- Food Session Cards - to replace those already supplied
- A1 Laminated Sheets - to replace the A1 Work pads previously supplied
- Replacement update packs for patient handbooks which contain 4 x Health profile and 4 x Action Plans.
Please email to the above address with each item you require and the quantiy, along with the appropriate delivery address(es) and we will organise for these to be sent out to you as soon as possible.
Items 1, 2 & 3 will be supplied for each resource pack you have in your locality
e.g. - if you have 3 sets of food models and magnetic board, will supply 3 sets of each.
Item 4 will be supplied for each patient handbook you have in your stock, please count all remaining stock & contact us with your count.
Please note only sites signed up to DESMOND are able to order resources.
