Local referral pathway to DESMOND locally: All newly diagnosed patients.
Courses run on a monthly basis with 2 per month in the F2 format (over 2 afternoons one week apart). Started DESMOND in August 2007 with 4 trained facilitators (1 dietitian and 3 nurses).
Providing the newly diagnosed programme to all new clients with type 2 diabetes in Co Galway. Referrals from all health professionals- acute and primary care.
