Local referral pathway to DESMOND locally: Practices refer directly to DESMOND co-ordinator via designated form or electronically via sci-gateway.
In this area Desmond is offered as 1st line education for people newly diagnosed with diabetes to empower them to become more informed about their condition and as a resource to assist management of the changes which diabetes will bring to their lives. It is also offered as a opportunity for people with newly diagnosed diabetes to meet and share experiences with both health care professionals and other people newly diagnosed with diabetes.
Our courses are run twice monthly across a variety of local venues and are delivered by three accredited Desmond trainers.
