Local referral pathway to DESMOND locally: Practices refer directly to Desmond administrator via designated form and the PCT standard is to offer patients to offer a date within 6 weeks of Diagnosis.


Overview

Desmond is offered to empower patients to become more informed about their condition and as a resource to assist management of the changes which diabetes will bring to their lives. It is also offered as a opportunity for people with newly diagnosed diabetes to meet and share experiences.


Our courses are run twice monthly across a variety of local venues and are delivered by three accredited Desmond trainers. G.P. Practices refer directly to a designated Desmond administrator and a future course date is offered usually within six weeks of diagnosis. The programme is evaluated via the designated Desmond criteria plus information to allow the DESMOND experience to be audited.
