Local referral pathway to DESMOND locally: Through health proffesional attached to GP practice ie GP, Practice Nurse or Dietician.
We provide courses over 2 half days or 1 full days at various venues accross Northumberland. These venues will be GP surgeries, church Halls or any venues able to accommodate a course. Various days of the week are used to facilitate attendance.
Desmond is funded by Northumberland Care Trust and patient with Diabetes type II are invited to come along with a partner to a course close to where they live after a referral is received from a professional within the GP practice.
Once the referral is received a letter is sent out with an invitation to the next available course, if this is incovenient every effort is made to find a course which is convenient.
Once the course has been completed information is given to the GP practice on attendance. We aim to make Desmond fun and informative within a relaxed atmosphere. If you want to make a difference to your health DESMOND is for YOU.
