Triaged through Diabetes Specialist Dietitian/Diabetes Specialist Nurse in specialist care.
The DESMOND programme has been in existence in the Wakefield District and North Kirklees Diabetes Network since April 2006. Patients are referred to the programme through a number of mechanisms, including being triaged by the Dietitian or Diabetes Specialist Nurse in specialist care and transferred onto the DESMOND programme and direct referrals from Practice Nurses/General Practitioners across the whole of the Diabetes Network.
Awareness raising of the programme is undertaken at every opportunity both with health care professionals and patients. This may be at, for example, multidisciplinary diabetes conferences/events, patient groups, public awareness sessions. To date, over 500 patients have been through the DESMOND programme, comments from those who have attended include: "This course enabled me to ask questions I didn't know I had! Armed with all the information I have acquired, I feel confident I can take control of my diabetes" "This excellent programme thoroughly discussed all elements of diabetes. It was very informative, well presented in an informal way"
