Local referral pathway to DESMOND locally: Referrals made via all 56 GP practices, referrals can be from other members of multidisciplinary diabetes team to 2 locality based Community diabetes support teams, appointments made through central Diabetes network Office.


Overview

The western Trust serves a population of around 300,000 people. The Trust is currently the first in Northern Ireland to implement the DESMOND programme. Our DESMOND educator training took place on 17/18th September 2009. Delivery of courses commenced immediately after this on 1st October 2009. A total of 12 members of community diabetes teams comprising of 6 diabetes specialist nurses and 6 diabetes dietitans will be delivering the programme across 3 counties. The programme will be delivered in a variety of hospital,primary care and council venues, and will be in both 1 full day and 2 half day formats.


We aim to deliver 30 newly diagnosed courses within the 1st year with a view to introducing additional courses for patients with established diabetes in the near future. The Western Trust has supported funding to implement the DESMOND programme for the next 3 years and has been actively committed to promoting models of patient self-management.
