Referral Pathway: Patients are contacted and invited to attend a session once referral is triaged by our service

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)

Overview
The DESMOND programme is held 3 times a month at Kingsley Hall, Parsloes Avenue, Dagenham by Diabetes Specialist Nurses and Registered Dietitians.

The DESMOND session starts at 9 am and finishes at 4.30 pm. Patients are welcome to bring along a guest. Refreshments are provided for both patients and their guest.

We also run BME DESMOND (ethnic minority groups) and aim to deliver 3 courses over the year across various sites in Barking and Dagenham.

The DESMOND programme has proved to be extremely popular and successful and feedback from participants has been very positive.
