Referral Pathway: GP or practice nurse will refer directly (written or electronic) to the service.

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND has been running in the Scottish Borders now for 4 years. We have a Team of 7 trained Educators currently-who deliver the Newly Diagnosed / Foundation modules. All our Educators have been quality assured.

At present clients choose the venue of their choice (5 localities across the region-Duns, Hawick, Kelso, Melrose & Peebles) and whether they wish to attend 1 x 6 hour session or 2 x 3 hour sessions. Sessions are very well received by clients. We also encourage our Practice Nurses, GP’s and other members of staff with an interest in Diabetes to come and sit in on a session to see what DESMOND is all about.

Referrals are made via GP, Practice Nurse, Lifestyle advisers, Dietitians either electronically or via written form. Ask your Practice for more information on DESMOND locally.
