Localities: DESMOND is currently offered in all areas of County Donegal for all people newly diagnosed with type 2 diabetes. Courses are delivered in each network are every 2-3 months or as demand dictates
Referral Pathway: DESMOND is discussed with patients by their nurse/GP at diagnosis. A referral form is sent by any health professional with the patients details and clinical information to the admin contact. Patients will be contacted and offered a course in their locality. Referral forms are available at the hospital diabetes clinics and in every GP practice. Additional referral forms can be obtained through Marie Carton Malone, dietetic admin
DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)
The Long Term Conditions project in Donegal funded the training of Health Professionals in Donegal to deliver DESMOND throughout the county and the project is supporting the sustainability of this programme in the initial two years.
The delivery of the Programme is being evaluated as part of the overall whole systems approach to Chronic Disease Management.
The Programme is delivered by a nurse and a dietitian in each network area who have trained as DESMOND Educators.
