Referral Pathway: via GP, Practice Nurse or Hospital Diabetes Team.

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

We have been Educators of the Newly Diagnosed/Foundation DESMOND course for over 10 years and usually run one course each month on Tuesdays, using both the F1 and F2 format. Our sessions are held in Dunchurch Sportsfield and Village Hall. People attending averages out at about 10 newly diagnosed patients from the Rugby district per session, with half of them bringing a partner with them. We have a waiting list of about three to six months.

Most of the people attending have been diagnosed with type 2 diabetes for about six months. DESMOND helps them to make sense of all the information provided at diagnosis and with the support of the group, work out ways to make better choices to enable them to manage their diabetes effectively.

Every new group raises different issues – so although we stick to the structured curriculum no session is ever the same and we’ve had very positive feedback from the people with type 2 diabetes, their partners and also from their Practice Nurses and GP’s.
