Referral Pathway: To enrol on a DESMOND programme you need to be registered with a Luton GP. Please ask you GP or Practice Nurse who will complete referral form and forward to Community Diabetes Services. When the referral is received a letter will then be sent to you requesting that you telephone to book an appointment.

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)
