Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available(for those with established diabetes)

The DESMOND programme has been in existence in the Wakefield District since April 2006. Patients are referred to the programme through a number of mechanisms, including being triaged by the Dietitian or Diabetes Specialist Nurse in specialist care and transferred onto the DESMOND programme and direct referrals from Practice Nurses/General Practitioners across the whole of the Diabetes Network. Self referral is now also possible, please contact Lorraine Humphrey for details.

Awareness raising of the programme is undertaken at every opportunity both with health care professionals and patients. This may be at, for example, multidisciplinary diabetes conferences/events, patient groups, public awareness sessions. To date, over 3000 patients have been through the DESMOND programme, comments from those who have attended include: “This course enabled me to ask questions I didn’t know I had! Armed with all the information I have acquired, I feel confident I can take control of my diabetes”
