Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available(for those with established diabetes)

Overview
A person with newly diagnosed Type Two diabetes is referred for DESMOND via their GP surgery or specialist service to the DESMOND co-coordinator.

DESMOND is an interactive session where everybody is encouraged to discuss their thought, feelings, questions and barriers in connection with diabetes. Its aim is to focus on the positive ways to manage your diabetes.
