Times available: 09.00 – 17.00 (Evenings, weekend and half days available on request)

Localities: DESMOND education programmes are delivered across the geography of the Northern Trust including Mid Ulster, Coleraine, Ballymoney, Ballymena, Antrim, Whiteabbey and Larne areas.

Referral Pathway: Referrals are accepted from GP, Practice Nurse, Dietitians, Diabetes Nurses, podiatrists, opticians, self-referral or any member of the multidisciplinary team.

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available(for those with established diabetes)

DESMOND was introduced into the Northern Trust in November 2015.

Every week we look forward to the group as each day is always unique and it’s wonderful meeting and listening to peoples’ experiences both positive and negative. The day is informal, including humour as well as the serious business.

All attendees are welcome to bring a relative or friend with them if they wish. It can be quite unnerving having to go to a full day of information and many people wonder what we will be talking about for six hours. However by the end of the day it all becomes clear. We have four regular Educators running the days and welcome allied health care professionals sitting in on DESMOND, using it as an education tool. We want them to see the messages DESMOND provides and it helps practice and community staff understand what information their patients are receiving.
