Referral Pathway: GPs or Practice nurses can refer to the Community Diabetes Team, Varndean Building, Brighton General Hospital, Elm Grove, Brighton, BN2 3EW

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)

DESMOND Walking Away from Diabetes module available

We currently run 2 DESMOND sessions per month, one at Moulsecoomb Leisure Centre and one at Portslade Health Centre. We also run one Walking Away from Diabetes session monthly and one Foundation module ( for those with established diabetes) monthly, at alternative sites. Feedback received from patients, GPs and Practice Nurses has been overwhelmingly positive DESMOND was initially set up as a pilot project for Brighton central locality GP practices in November 2007.

Participants learn more about diabetes, food, medication and how to monitor and manage their own condition. They are given the opportunity to discuss their experiences with others and there is plenty of opportunity to ask questions in an informal and relaxed atmosphere. Participants are welcome to bring a friend or relative with them. Tea & coffee will be provided.
